PIERCING CONSENT FORM
Please ensure you fill out the consent form
ON THE DAY of your piercing appointment at Teddington Ink.
Age-Based Piercing Requirements:
- 10-12 years: Ear lobes ONLY - A parent or legal guardian must be present & co-sign the consent form.
- 13-16 years: Ear lobes & Nostrils - A parent or legal guardian must be present & co-sign the consent form.
- 16+ years: All piercings except nipple piercings.
- 18+ years: All listed piercings.
Identification Requirements:
- Under 25: Clients under 25 must present physical ID. Photocopies or photos of ID are not acceptable.
- Under 16: A parent must present photo ID with the same surname or provide additional ID that verifies the relationship (e.g., birth certificate). Legal guardians must bring photo ID and proof of guardianship.
Appointments cannot proceed without a properly completed consent form.
**Please note, if you answer “yes” to any health-related questions on our consent form we may require a doctor’s note before proceeding with your appointment. This policy ensures that any potential health risks are addressed to protect both you and our practitioners. Your safety is our priority, and obtaining medical clearance helps us provide the highest standard of care.

By signing the consent form, you agree to the following:
- You have had the opportunity to ask questions and received satisfactory answers about the piercing process.
- You understand the potential risk of allergic reactions to the jewellery or procedures.
- You acknowledge the risk of infection if aftercare instructions are not followed and agree to adhere to them.
- You confirm you are not unwell, have no COVID-19 symptoms, and have not been exposed to COVID-19.
- You understand that piercings can result in permanent changes such as scarring.
- You confirm you are over 16 or have a parent/legal guardian present if under 16, and that the decision to get pierced is yours.
