Tattoo Appointment Consent Form Guidelines
Please ensure you fill out the consent form on the DAY OF your tattoo appointment at Teddington Ink.
Identification Requirements:
If you look under 25, don't be offended if you are asked for ID. We are obligated to comply with the law and don't take chances. Clients under 25 must present physical ID. Photocopies or photos of ID are not acceptable.
Appointments cannot proceed without a properly completed consent form.
**Please note, if you answer “yes” to any health-related questions on our consent form we may require a doctor’s note before proceeding with your appointment. This policy ensures that any potential health risks are addressed to protect both you and our practitioners. Your safety is our priority, and obtaining medical clearance helps us provide the highest standard of care.
Consent Form Acknowledgements:
By signing this consent form, you confirm that you have read and fully understand the following:
- You have had the opportunity to ask questions and received satisfactory answers about the tattoo process.
- You agree to follow all care instructions provided by the tattoo artist.
- That tattooing involves piercing the skin with a needle and depositing ink, which may cause pain, discomfort, swelling, and bleeding.
- There is a risk of an allergic reaction to the ink.
- You have informed the tattoo artist of any known allergies or medical conditions that might affect the tattoo process.
- The tattoo artist has explained the risks of infection associated with tattooing and provided proper aftercare instructions.
- The tattoo may not be completely removable, and attempts to remove it could result in scarring, skin discolouration, or other complications.
- The final appearance of the tattoo may differ from the initial design due to factors such as skin texture, healing, and the skill of the tattoo artist.
- You release the tattoo artist from any liability, including but not limited to injury, infection, allergic reactions, or damage to personal property that may arise from the tattoo process.
- You understand that the tattoo artist is not responsible for any medical expenses or other costs resulting from complications related to the tattoo.
- You certify that you are 18 years of age or older and have provided valid identification.
- You acknowledge that you have read and understand this client consent and liability form in its entirety and agree to its terms.
